چکیده

BACKGROUND The right subclavian artery, originating not from the brachiocephalic trunk but directly from the aortic arch, is a rare anatomical abnormality. 'Arteria lusoria' is the accepted name of the retroesophageal right subclavian artery. Such a vessel location, between the vertebral column and the esophagus, determines its course to the right. This defect may be asymptomatic, found during autopsy or coincidentally during diagnostic procedures. However, it may also be symptomatic. The course of this major blood vessel in the limited anatomical space may cause symptoms of mediastinal organ compression. The aim of this paper was to present two cases of abnormal anatomical course of the right subclavian artery and its aneurismal dilation. In this study, CT scans of the saccular aneurysm of the retroesophageal right subclavian artery were used: of a male patient diagnosed at Euromedic Diagnostics in Olsztyn and of a female patient, from the resources of the Radiological Dept. at MSWiA Hospital in Olsztyn CASE REPORTS An 85-year-old female was admitted to Hospital ER for congestive heart failure decompensation. Her chest X-ray revealed a round mass in the upper right mediastinum. Chest CT confirmed the presence of a saccular aneurysm of the lusory artery, 6.5 cm in diameter, partially filled with thrombotic material. The patient died in hospital from myocardial infarction that was not related to the aneurysm. A 61-year-old male patient had a chest X-ray which showed a round opacity on the apex of the right lung. The diagnostic process comprised also chest computed tomography. The examination showed an anomalous origin of the right subclavian artery, with aneurysmal dilation and compression of the oesophagus and of the trachea. An intraluminal thrombus was found. The patient remained under observation till the next CT examination, 6 months later. CONCLUSIONS The presented rare cases of arteria lusoria aneurysm are not only casuistic reports. They also show the value of modern MDCT diagnostics. Multi detector CT examinations allow for a proper evaluation of the vascular anomalies in mediastinum. The analysis of vascular pathologies and accompanying compression of adjacent organs makes further treatment possible.

Endovascular treatment of an aberrant right subclavian artery aneurysm with use of the Zenith iliac plug.

Aberrant right subclavian artery aneurysms are rare but demonstrate a propensity toward rupture; thus, early elective repair is mandated. Novel endovascular modalities are available to offer less-invasive treatment for a patient population with increasing comorbidities. We report a case of a 65-year-old woman with an aberrant right subclavian artery aneurysm causing dysphagia lusoria. The aneur...

Single-stage surgical repair in a complex case of aberrant right subclavian artery aneurysm and common carotid trunk

Aberrant right subclavian artery with coexisting common carotid trunk is an extremely rare congenital anomaly affecting <0.1% of the population. We report the case of a 77-year-old Caucasian man presenting with dysphagia and dyspnea secondary to an aberrant right subclavian artery aneurysm and describe our technique for open surgical repair.

A Retroesophageal Right Subclavian Artery as the Last Branch of the Aortic Arch- In Vivo Case Report

Three major arteries normally originate from the aortic arch. Variations of the aortic arch branches are observed in some cases. Rare variation of the aortic arch branches is originated from the retroesophageal right subclavian (RRS) artery as the last branch of the aortic arch. Variation of artery is possibly associated with the variation of the recurrent laryngeal nerve. This research p...

Surgical treatment of an aneurysm of the aberrant right subclavian artery involving an aortic arch aneurysm and coronary artery disease.

A 55-year-old man presented with clinical signs of an aortic arch aneurysm. Angiography, MRI and CT demonstrated an aortic arch aneurysm and an aneurysm of the aberrant right subclavian artery. Coronary angiography revealed 95% stenosis in the right coronary artery. Right common carotid artery-right subclavian artery bypass, arch graft replacement and coronary artery bypass grafting were perfor...

Pseudocoarctation with saccular aneurysms, left sided SVC and aberrant right subclavian artery - A case report.

Pseudocoarctaion is a rare congenital anomaly due to elongation of aortic arch. The exact etiology is still uncertain. It may be associated with other congenital cardiac and vascular anomalies. We report an unusual case of pseudocarctation associated with aberrant right subclavian artery, left SVC and multiple saccular aneurysms in the kinked arch and we feel that this is the first documented c...

The Case Report of Variation in Subclavian Branches

The Subclavian artery is a major branch of the Aorta. In the left the subclavian artery arises from aortic arch, and in the right it arises from the Brachiocephalic trunk This artery passes from the subclavian groove on the first rib and continues as Axillary artery. The Subclavian artery has several branches that supply chest wall, Thyroid gland and cervical region. Several variations about th...
